Unauthenticated Users Panel v1.13

**** No Need to Install New Version If You Have v1.12!!! ****

There's a slight addition to the Unauthenticated Users Panel, all those that have version 1.12 are NOT required to upgrade cause the change is not in functionality of the infusion. The change is that I included a infusion.php file to allow the installation of the infusion via the normal means of PHP Fusion.

Also updated the Read Me file to include installation and removal instructions of the infusion. The infusion can be added by both methods, 1) the old one via _install.php and 2) the php fusion standard method via infusion.php

The difference between the two methods is that the old method added the icon to the Users Admin panel, whereas the php fusion standard method adds it to the Infusions panel.
